Abstract

저자는 자해행위에 의한 악안면 부위의 총상 환자 7명을 대상으로 전산화단층사진 검사를
통해 다음과 같은 결과를 얻었다.
1. 총탄의 입사구는 모두 이 하부(submental area)였고, 출사구는 콧등이 3예, 전두부, 안
와, 안와하부위, 비익외 측부위가 각각 1예 였다. 입사구는 1 ㎝내지 3 ㎝의 직경을 가진 원
형이며, 출사구는 입사구에 비해 크기가 크고, 길쭉한 형태 (가로길이<세로길이)였다.
2. 총탄파편은 2예에선 관찰되지 않았고, 5예에선 안면부위에 산발적으로 흩어져 있었다.
3. 7예 모두에서 하악전방부의 골절 (그중 5예에서 분쇄골절)을, 1예에서 하악체의 분쇄골
절을 보였고, 상악골은 7예 모두에서 분쇄골절 (상악 전방부 5예, 상악골체 2예)을 보였다.
모든 경우에서 상악치아의 파절을 보이며, 3예에서 하악치아 파절을 보였다.
4. 6예에서 비골의 분쇄골절과 비중격의 골절을 보였다.
5. 부비동의 포함여부는 7예중 상악동의 골절이 5예, 사골동의 골절이 4예, 전두동의 골절
이 2 예 순이었고, 이들 골절된 부비동은 모두 공기 -유체액 레벨(air-fluid level)이나
non-aera-pion양상을 보였다
6. Supralaryngeal airway는 1예를 제외한 6예 에서 막혀 있었다.

The purpose of this study was to evaluate the clinical and computed tomographic
features of 7 cases of maxillofacial gunshot juries in the suicidal patients visited the
emergency room, Capital Armed Forces General Hospital.
The obtained results were as follows :
1. The gunshot wounds were directed from submental area to dorsum of nose(3
cases), frontal area(1 case), orbit(1 case), infraorbital area(1 case), and lateral to nasal
wing(1 case). The shape of inlet in gunshot wounds were round (diameter 1∼3㎝m) and
that of outlet were oval shape(size : inlet2. The bullet fragment wasn't found in 2 cases, but in the other 5 cases, the bullet
fragments were showed in facial
3. All Gases showed the fracture on anterior mandible (5 cases : comminuted
fracture), and 1 case showed the comminuted fracture on mandibular body. The maxilla
were comminutedly fractured in all cases (5 cases : anterior maxilla, 2 cases maxillary
body). In all cases, the fracture of maxillary teeth were showed and in 3 cases the
fracture of mandibler teeth were showed.
4. The comminuted fracture on nasal bone and fracture on nasal septum occurred in 6
cases.
5. The cases of injuries on paranasal sinus were 5 cases of fracture on maxillary
sinus, 4 cases of fracture on ethmoidal sinus, 2 cases of fracture on frontal sinus. This
every fractured paranasal sinus showed air-fluid level or non-aeration state.
6. Obstruction of supralaryngeal airway occurred in 6 cases.
